Partner with post-acute facilities to heal wounds and prevent avoidable hospital admissions. Wounds are followed over weeks and months by the same physician, so you see the results of your own decisions.
Become an Expert Wound Care Clinician
Crescent runs its own wound care certification programme, awarding 30 CNE contact hours. Clinicians who join us keep access to it, and to the in-service teaching we deliver to facility teams.
Post-Acute Care
Treat complex and challenging chronic wounds at skilled nursing facilities, long-term acute care hospitals and assisted living facilities — at the bedside, in a setting the patient already knows.
Clinical Judgment Comes First
Treatment decisions rest with the physician who examined the patient. Procedures and advanced therapies are used when they are right for that wound and that resident, not because they are available.
Reduced Administrative Burden
Our own wound care EMR and administrative support keep the paperwork short. Notes are completed on the day of the visit and shared with the facility, so nothing follows you home.
