    Physicians complete clinical notes on the day of the visit, documenting assessments, treatment plans, procedures, and follow-up recommendations. Documentation is promptly shared with the facility through EMR systems and integrations.

    Physicians actively communicate with nurses, DONs, and care teams about significant findings, treatment changes, and concerns. Facilities also have a clear channel to reach Crescent between scheduled rounds for questions or issues.
