Treatment is tailored to each resident’s condition, wound characteristics, and response to prior care rather than applying a one-size-fits-all approach. Advanced therapies are used only when clinically appropriate, not simply because they are available.

Crescent maintains consistent physician coverage so the same clinician follows wounds over weeks or months. This continuity allows the physician to understand the wound’s history, monitor changes, and adjust the plan without repeatedly starting over.

Wound care is delivered within the facility whenever clinically appropriate and safe. This reduces unnecessary outside appointments while allowing residents to receive care in a familiar environment with their facility team present.

Physicians complete clinical notes on the day of the visit, documenting assessments, treatment plans, procedures, and follow-up recommendations. Documentation is promptly shared with the facility through EMR systems and integrations.

Physicians actively communicate with nurses, DONs, and care teams about significant findings, treatment changes, and concerns. Facilities also have a clear channel to reach Crescent between scheduled rounds for questions or issues.

Physicians remain involved and accountable for the wounds they manage even after leaving the building. They provide clinical context when questions arise about their treatment decisions or documentation.

Crescent provides regular wound care education and in-services for facility staff, focused on practical SNF issues. Contracted facility nurses also receive complimentary access to Crescent’s wound care certification program.
